    Seronegative rheumatoid arthritis and plaquenil

It is one of two main types of rheumatoid arthritis diagnoses. In most cases of rheumatoid arthritis, the patient tests positive for rheumatoid factor (RF) and/or anti-citrullinated peptides (CPP) antibodies.

Hydroxychloroquine is in a class of medications that was first used to prevent and treat malaria. Today, it is used to treat rheumatoid arthritis, some symptoms of lupus, childhood arthritis or juvenile idiopathic arthritis and other autoimmune diseases. It is not clear why hydroxychloroquine is effective at treating autoimmune diseases. Rheumatoid arthritis is a chronic disorder for which there is no known cure. Fortunately in the last few years, a shift in strategy toward the earlier institution of disease modifying drugs and the availability of new classes of medications have greatly improved the outcomes that can be expected by most patients. Plaquenil is a DMARD which has been prescribed for years to treat rheumatoid arthritis, lupus, juvenile rheumatoid arthritis, and other autoimmune diseases. When the first biologic DMARD received FDA-approval in 1998, and as other biologics followed in subsequent years, it appeared as though Plaquenil would become less prescribed.

These patients tend to have a more severe disease course with more joint deformities, x-ray damage, disability and inflammation outside of the joints. These indicate that the patient is seropositive and that they possess the antibodies that cause an attack on joints and lead to inflammation.

But the diagnostic criteria are such that even those without positive blood tests, the so-called “seronegative” cases, should still receive a diagnosis and treatment. Hopefully researchers will develop more accurate blood tests that will be able to detect rheumatoid arthritis much earlier so that preventative treatments can begin. Long story shorter, my diagnosis is still seronegative inflammatory poly arthritis. The most important issue for me is that I have been treated aggressively and joint damage is still minor because of that. I am currently on methotrexate, Plaquenil and Humira with prednisone bursts for flares. I'll try to make this short. I'm 38, have rheumatoid arthritis otherwise, in excellent health. I have heard/read horror stories about this disease and have my own unfortunate stories but I want to tell EVERYONE about the medication my doctor prescribed.
